The office is Suite 2800 in the Rush Medical Office Building on the Rush Oak Park Hospital campus, on Maple Avenue between Madison Street and Jackson Boulevard. Parking is free: there is a garage on Wisconsin Avenue on the east side of the hospital and surface lots on Maple Avenue, and free valet parking is offered on weekdays at the Maple Avenue and Monroe Street entrance. Take the elevator to Suite 2800. By train, the Oak Park stop on the CTA Blue Line is a few blocks away.
