The office is Suite 360 in Chalet Office Plaza, a five-story office building at the corner of Lake Avenue and Skokie Boulevard, just off the Edens Expressway (I-94). Westbound, take exit 34C (Lake Avenue east) and turn onto Skokie Boulevard; eastbound, take exit 34A (Skokie Boulevard / US-41) and continue south. Take the elevator to the third floor.
